What Services Do We Provide?
Healing Hands provides comprehensive primary care services, including physical exams, advanced care planning services, screenings, labs, medications, diagnostic testing, and referrals to specialty care. Additionally, Healing Hands offers disease management services for patients with multiple chronic conditions, such as diabetes, hypertension, and asthma.
Services include consistent care, regular screenings, a coordinated health care plan, support management, patient education/coaching, and access to specialty care. Care management focuses on empowering lifestyle changes, like nutrition, weight loss, smoking cessation, and exercise.

Education
Promoting self-reliance in patients has always been a part of the full patient experience at Healing Hands. Through preventative screening, physical evaluation, education and case management, our providers work to empower patients to take control of their health. Patient education is particularly essential for those patients with chronic conditions. The goal being to motivate individuals to connect to regular primary and preventive care, thereby alleviating inappropriate use of hospital ERs that result from deferred diagnosis and treatment.
Referral Services
For services that we are unable to provide ourselves, we are pleased to provide referrals. This includes but is not limited to: Cardiology, Pulmonology, Home Health, Palliative, Hospice, Diagnostics and Lab Services.
